A penile implant surgery is a very well-known and effective solution to restore sexual power in medium to extreme cases.

Malleable Penile Implant:
It consists of two pieces made of medical silicone rubber reinforced with metal cores on the inside. This structure provides rigidity while maintaining the necessary softness to bend the rods downward when sexual intercourse is not desired, and upward when it is.
The malleable penile implant is placed inside the penis in place of the cavernous tissue. It allows the patient to have a permanently erect penis, unlike the inflatable penile implant, which lets you control when to achieve an erection.

This option might seem simpler, easier, and even more affordable. However, since this implant remains rigid at all times, it can sometimes be embarrassing or at least uncomfortable, as the penis stays in an erect position permanently.
This type is recommended for patients with neurological diseases, paraplegia, or total paralysis who cannot use their hands with sufficient strength. It is also suitable for patients over 70 years old who may not have the physical ability to operate an inflatable implant, or for patients with a penis size of less than 7 cm. These are the only cases for which we recommend a flexible (malleable) implant.

Otherwise, for patients who do not suffer from any neurological diseases and have sufficient hand strength and appropriate penis size, there is no need to consider implanting a malleable penile implant. We do not recommend this type for such cases.
3-piece Penile Implant:
The three-piece inflatable penile implant is the most commonly used worldwide. There are two main versions: the first increases the penis width only, while the newer version increases both the width and length with repeated use during sexual intercourse.
Three-piece inflatable penile implants consist of a fluid-filled reservoir implanted under the abdominal wall, a pump and a release valve placed inside the scrotum, and two inflatable cylinders inside the penis.

To achieve an erection, you pump the fluid (salt water) from the reservoir into the cylinders. Afterwards, you release the valve inside the scrotum to drain the fluid back into the reservoir.
One of the main advantages of the three-piece inflatable penile implant is the ability to control erections. Since an erection is not always needed, it’s much better to have a discreet tool that allows you to achieve it only when desired. Steps of Penile Implant Surgery in Turkey
We will now explain the three stages of penile implant surgery for the treatment of erectile dysfunction:
1 – Before the operation
The patient is advised to control diabetes and ensure that the HbA1c (cumulative sugar test) result is not more than 10 before coming to Turkey. It is also important to inform us of any health conditions, such as heart, kidney, or other diseases. We may ask for additional tests beforehand to ensure the patient is fit for surgery.
Stop taking blood thinners such as aspirin and Plavix at least 5 days before the operation.
Do not shave the pubic hair; a specialized staff member at the hospital will take care of this to avoid any accidental cuts or wounds.
Two days before the operation, the patient should keep the pubic area clean using a specialized antiseptic soap to reduce bacteria before surgery.
2 – During the procedure
Penile implant surgery in Turkey for the treatment of erectile dysfunction is performed in a specialized operating room, the first of its kind in the country, designed to meet the highest sterilization standards used in organ transplantation procedures.

The patient receives either local or spinal anesthesia, depending on the anesthesiologist’s decision, along with intravenous antibiotics. The skin is then sterilized with special antiseptic solutions and carefully isolated with sterile drapes to minimize the risk of infection.
The implant can be inserted using two main surgical approaches:
Penoscrotal approach:

This is the most commonly used method. A small incision of approximately 2 cm is made at the junction between the penis and the scrotum.
Through this opening, the entire implant is placed, including the cylinders, pump, and reservoir. This technique provides excellent exposure of the corpora cavernosa and allows for precise placement, with a scar that heals quickly and becomes almost invisible within a month.
Infrapubic approach:

Both techniques are safe and effective, and the choice of method depends on the patient’s anatomy, medical history, and the surgeon’s preference. Regardless of the technique, the surgical wound is small, heals within weeks, and leaves minimal to no visible trace.
3 – After the operation
Painkillers and antibiotics are prescribed for two weeks after the operation. The stitches used at the wound site are absorbable and do not need to be removed manually. Most patients are discharged from the hospital on the second day after surgery.
The patient is advised not to walk long distances during the first week after surgery, and showering is prohibited for the first 5 days.
When an erection stent (penile implant) is placed, we advise the patient to apply an ice pack on the scrotum to minimize swelling in the operated area.
We also ask the patient to bring more than one pair of tight underwear. These help keep the penis in a specific position after surgery, which we explain in detail upon the patient’s arrival.

So, a patient with ejaculation issues or low libido should look for another solution apart from the penile implant.
On the contrary, if the patient has normal ejaculation and sexual desire, the penile implant does not affect these functions at all.
The penile implant can give a man a strong erection and help him have an enjoyable sexual relationship if he has the libido, but it will not create the sensation if it doesn’t exist naturally.
Penile Implant Surgery in Turkey:

Even though it’s a surgical procedure, the penile implant is considered one of the simplest types of surgeries. The incision is no longer than 2 cm (0.7 inch), located in the area right between the penis and the scrotum.

The most important question is: Do all cases of ED require a penile implant?
The answer is NO. However, anyone suffering from erectile dysfunction who does not respond to medical treatment may require a penile implant.
In many cases, ED is due to chronic conditions such as nerve and peripheral vascular damage associated with diabetes and atherosclerosis. More recently, penile implants have also been recommended for certain psychological causes of ED that are unresponsive to pharmaceutical treatment.
Penile Implant Brief History
In the 16th century, French scientist Ambroise Paré made the first attempt to create an artificial penis using wooden tubes to facilitate urination for a specific patient.
The first attempt to implant material into the penis tissue was in 1936 by Russian physician Nikolaj Bogoraz, who used rib cartilage and implanted it to provide rigidity during erection.
This method was not ideal, so new alternatives were sought. In 1950, American doctor Dr. Scardino used the first synthetic material for implantation. In 1952, Drs. Goodwin and Scott published results using an acrylic material placed under Buck’s fascia (the inner fat layer of the penis).
In 1960, Drs. Loeffler and Sayegh published results of using the same acrylic material but implanted inside the cavernous tissue rather than outside it.
In 1966, Egyptian plastic surgeon Dr. Beheri pioneered the use of polyethylene rods placed inside the cavernous tissue under the tunica albuginea. He operated on 700 cases using this technique, achieving strong, durable erections.
The first inflatable penile prosthesis appeared in 1973 by Dr. Scott and colleagues. The device consisted of three parts, and erection was achieved by pumping fluid from a reservoir into two cylinders inside the penis. After intercourse, pressing the pump again would return the fluid and restore flaccidity.
Since then, two main types of penile implants have remained: the inflatable prosthesis, which has significantly evolved and dominates the American market, and the malleable prosthesis, which remains more affordable and widely used in Turkey, the EU, and the US.

Since the operation is performed under anesthesia, there will be no pain during the operation. As this is still a surgery, some feelings of pain might be after the operation which can be treated with painkillers.
You can be discharged the same day after penile implant surgery. Recovery occurs after an average of 4 -6 weeks.
In case of not working in a physical job after, in 2 weeks you can return to your job. If you have a heavy physical job, it is recommended to get a 30-day rest.
After 2 weeks, you can start physical activity and do light sports exercises. However, you should avoid heavy training, wait at least 30 days for intense exercises, and get your doctor’s approval.

And therefore, because the erection can be done manually, it is not likely to experience erection problems at all any time after the operation, any premature ejaculation decreased sexual desire, and/or any other functional problem around the penis area.
Penile implant surgery can fix curvature in the penis in its early stages. Like if before the surgery, if the penis was slightly curved during erection it can be cured with no more issues of any curvature in the future,
but in case of having a big curvature, additional surgery might be needed to cut any part of the skin that holds it together and causing the curvature.
No, the urinary tract is in no way affected by penile implant surgery.
No, penile implant surgeries do not affect reproduction in any way.
